Your Health Information Rights
You have the right to:
Inspect and obtain a copy of your medical records
Request amendments to your records
Receive a list of certain disclosures we have made of your PHI
Request restrictions on how we use or share your PHI (when legally possible)
Receive confidential communications by alternative means (e.g., at a different address)
Our Duties
Profusions is required by law to:
- Maintain the privacy of your PHI
- Provide you with this Notice of Privacy Practices
- Notify you if a breach of your PHI occurs
- Follow the terms of this Notice currently in effect
How We May Use and Disclose Your PHI
We may use or disclose your PHI for the following purposes:
- Treatment: To provide, coordinate, or manage your care with other healthcare providers.
- Payment: To bill and obtain payment for services rendered.
- Healthcare Operations: To support daily operations such as quality improvement and staff training.
- As Required by Law: To comply with federal, state, or local requirements.
Other Uses of PHI
Any other use or disclosure of your PHI will be made only with your written authorisation. You may revoke your authorisation at any time in writing.

If you believe your privacy rights have been violated, you may file a complaint with:

- U.S. Department of Health and Human Services (HHS), Office for Civil Rights.
We will not retaliate against you for filing a complaint.
